How to Create a Scored Assessment

After selecting the SCORED ASSESSMENT option:

  1. Enter the assessment’s title and description in the appropriate boxes. Use the tools on top of the text box to change the font style, add bulleted or numbered lists, insert links, photos or videos, and more.
  2. Insert an image by dragging and dropping the image file into the IMAGE box. Accepted files are PNG, JPG or GIF. A high-resolution image that is at least 700 px wide with a 4:3 ratio is recommended.
  3. Select one of the following two options at the bottom of the page:
  4. To collect the personal information of those completing the assessment, click on COLLECT PERSONAL INFORMATION and check the boxes next to the specific information you need.
  5. Click SAVE AS DRAFT in the top-right corner of the page to save your assessment.

    1. Once you click SAVE AS DRAFT, a CREATE A SECTION button will appear at the bottom of the page.
  6. Insert the section title, description and image in the appropriate boxes. Then, click CREATE in the bottom-right corner.

    Note: Select + ADD A TIP under the image box if you’d like to add any additional information to this section.
  7. Click CREATE A QUESTION at the bottom of the page.
  8. Add your question in the text box and click NEXT at the bottom of the screen.

    1. Select an ANSWER TYPE from the drop-down list: SINGLE CHOICE, MULTIPLE CHOICE, LINEAR SCALE. (See the Adding a Question by Type section below for instructions on how to add each type of answer to a question.)

Note: The + ADD IN BULK option allows you to add all the answer choices for this question as a group. 

Adding a Question by Type

To create a MULTIPLE CHOICE question:

Multiple Choice: The user can choose several answers from a list of possible answers.

  1. Select MULTIPLE CHOICE from the drop-down list and click on + ADD A CHOICE.
  2. Add the first answer in the text box below labeled A. Then, click the PLUS (+) sign on the right of the box to add another answer (or select the TRASH icon to delete it).

    Note: Select + ADD A TIP under the text box if you’d like to add any additional information to this choice.
  3. At the bottom of the screen, select NEXT to go to the next step.

    (Select PREVIOUS to go back to the previous step, SAVE & CLOSE to save your work and close return to the main menu, or DISCARD CHANGES to delete your answers.)
  4. Assign points to each possible answer in the POINTS tab. This will help generate a result at the end of the assessment. Then, press NEXT.
  5. Add some feedback for each possible answer, if necessary. Then, click NEXT at the bottom of the page.
  6. Check the box next to THIS QUESTION IS REQUIRED to make this question mandatory. (Leave it blank if it is optional.) Then, click SAVE & CLOSE at the bottom of the page.
  7. To add another question, select CREATE A QUESTION at the bottom and choose the order placement of the next question (before or after the last question saved). Then, repeat the steps above.

To create a SINGLE CHOICE question:

Single Choice: The user can only choose one answer from a list of possible answers.

  1. Select SINGLE CHOICE from the drop-down list and click on + ADD A CHOICE.
  2. Add the first answer in the text box below labeled A. Then, click the PLUS (+) sign on the right of the box to add another answer (or select the TRASH icon to delete it).

    Note: Select + ADD A TIP under the text box if you’d like to add any additional information to this choice.
  3. Follow STEPS 2-5 above.
  4. To send users to a different question or section depending on their answer, select the appropriate questions/sections from the drop-down list for each answer. Then, click NEXT.
  5. Check the box next to THIS QUESTION IS REQUIRED to make this question mandatory. (Leave it blank if it is optional.) Then, click SAVE & CLOSE at the bottom of the page.

To create a LINEAR SCALE question:

Linear Scale: Provide a numerical range for users to choose from.

  1. Select LINEAR SCALE from the drop-down list, assign a lowest and highest possible score, and label each. Then, click NEXT.
  2. Assign a point value to each answer. Then, click NEXT.
  3. Add some feedback for each possible answer, if necessary. Then, click NEXT at the bottom of the page.
  4. Check the box next to THIS QUESTION IS REQUIRED to make this question mandatory. (Leave it blank if it is optional.) Then, click SAVE & CLOSE at the bottom of the page.

Adding Scores and Points

This section allows you to provide users with results from their assessment or quiz.

  1. Select SCORE AND POINTS in the left-hand menu, and click ADD A RANGE in the second column.
  2. In the appropriate boxes, create a score breakdown and give users feedback based on their final score.

    Note: Your maximum possible score will always be displayed at the top of this column, as seen below:
  3. To add the next range, select ADD A RANGE and follow STEP 2 again.
  4. Click SAVE AS DRAFT in the top-right corner when you are done.

Publishing an Assessment

  1. Press the BACK ARROW at the top of the first column.
  2. Click the THREE DOTS in the right-hand corner of the third column, then select PUBLISH from the drop-down list.
  3. Click CONFIRM in the pop-up window.

And you’re done!

Your new assessment has been published and is accessible by app users. It will instantly appear in the ASSESSMENT & QUIZ module of your app like so:
